What happens when I call Square One Health?
When you call our toll free number, (855) 886-9511, you will first need to choose which one of our 3 locations you are interested in. From there you will choose option 1 for admissions. One of our admissions coordinators will answer the phone and help with any questions you may have. We will begin with an initial phone assessment to assess the client’s needs to determine which level of care would be appropriate to admit the client to according to ASAM criteria. If criteria is not met for any of our services, our treatment team will work with the client to make outside referrals with other local health agencies in order to help the client get the help that he/she needs.
What happens when I arrive?
Once admitted to our program, a member of our clinical staff will schedule a more detailed assessment of the individual to develop a custom treatment plan. The evaluation may include:
- Drug and alcohol history of abuse
- Medical information/history (psychiatric, physical, and neurological)
- Urine Analysis
- Criminal history
- Prior programs history
- Consent to release confidential information
- Client contact list
- Medical Assessment
